Verification Requirements

Food Assistance Hours 
Tuesday:         9 am to 2:30 pm
Wednesday:   11 am to 4:30pm
Friday:             9 am to 2:30 pm

Every household member over the age of 18 (excludes students)

• Copy of valid Michigan Drivers license or valid other state ID
• Current piece of first class mail with name and address. ( we can also accept car registration or voter ID)
• Verification of income for each household member earning money. (any one of the following)
  • Pay stub
  • Income tax showing gross income
  • Bank statement showing deposit of income
  • Unemployment statement or Social Security statement

For children under 18 please provide at least one of the following documents.

*Form MUST contain child's name and current address

-School records*
-Health Insurance Card*
-Print out of Immunization Record (hand-written copies not accepted)*
- Medical Record*
- Court Document*

It is also  recommended that any infants (under 1 year old) that you bring a copy of their birth certificate for verification.
